Select from one of our three packages listed above and download a PDF of our camp application. Complete all pages of the application, sign and mail with the required deposit to the following address:

Once we receive your application, and it has been reviewed, a confirmation packet and invoice will be mailed to verify your enrollment. Once you are enrolled we will provide the additional details that you will need get the most out of your Phantasy as well as an invitation to Camper Orientation at Citizens Bank Park.
